[ST] [SA] Debrief Meeting Agenda

The restriction and the discipline of what to say at the debrief is mostly
on the people on the receiving end of the comments, not so much on the
people who have things they need to express.

It's very easy in the heat of the moment to want to justify yourself or
explain why something happened or brainstorm a better way.  In the span of
time we've got before everyone loses patience, the only responses we CAN
make are "Thanks, I'm writing that down" or "Yes, we need to follow up on
that outside the meeting," or in certain cases, as necessary, "Oops, that
was a mistake, and I'm sorry."

Good note-taking is very important for the debrief, which should include
marking those things that need followup or additional actions.
